The team at Level Up is excited to share with you this comprehensive guide to establishing and managing an Ohio Health Transitions Program (OHTP)in your county. A program of the Ohio Department of Mental Health and Addiction Services, OHTP, funded through a grant from SAMHSA, works to improve access to treatment and support services for youth and young adults ages 16-25 with a serious emotional disturbance (SED) or a serious mental illness (SMI); Level Up is the outreach component of the program. The program was launched in Cuyahoga and Lorain counties but a key component of the project is to ensure that counties across the state of Ohio could learn from our experience and develop programs for their regions. From initial conceptualization to operational management, this handbook is tailored to equip you with the necessary tools and insights to successfully navigate the intricacies of establishing and maintaining a Healthy Transitions Project. This handbook serves as your compass, guiding you through the processes and strategies essential for the successful initiation and progression of your program.
